Learner Consent to Information Sharing (collected by InStride through the corporate partner online experience):

I understand that I will be participating in an education benefit program (“Program”) sponsored by [Company name] (“Company”) and administered by InStride.

I give my consent to Weber State University to disclose the necessary personally identifiable information from my education records (“PII”) to Acadeum and to InStride, for the purposes of InStride managing the program on behalf of Company, ensuring my program eligibility through Company and participation in the Program, confirming my enrollment, and facilitating payments to Weber State University. I waive my rights under Utah Code 53B-28-101 et seq.

I further understand and agree that my PII may be redisclosed by InStride to Company, and to InStride’s third party contractors or subcontractors for the limited purpose of carrying out InStride’s scope of responsibilities for providing the Program or its benefits.

The PII I authorize Weber State University to disclose includes any record(s) directly related to me as a student and maintained by Weber State University that are necessary to disclose for the purposes listed here. This may include any information or data recorded in any medium, including, but not limited to: handwriting, print, computer media, video or audio tape, film, microfilm, and microfiche.

PII that Weber State University may share under this provision includes: name, Student ID, contact information (including address, email address, telephone number), Company name and employee ID (if relevant), admission, registration and enrollment activity, academic standing, grades, graduation date, program and course names and credit hours, and program-related financial information (including tuition and fees billed and owed, scholarship information, account balances, billing statements).

I understand that I must provide this consent in order to receive the benefits of the Program, including tuition payments from Company. I further understand that I am not required to consent to the disclosure of my PII as a condition for independently enrolling as a student at Weber State University and receiving Weber State University educational services.

If I do not wish to provide this consent, I understand Company will not allow participation in the Program and I may apply directly for enrollment to Weber State University. To the extent the terms in this provision (“Consent Provision”) are inconsistent or in conflict with the terms of any other policy or agreement from or between me and Weber State University or InStride, the terms in this Consent Provision shall control.

Learner Financial Responsibility Agreement

I understand and agree that if I enroll as a Weber State University student I will ultimately be responsible for all tuition and fees associated with my class enrollment. Thus, I understand that if Company/sponsor fails to make a payment, that portion of my tuition and/or fees will be billed to my student account and I will be responsible for payment of those charges by the billing statement due date.
